This intermediate-advanced course will provide a systems-based approach to assess the premature/medically complex infant in the NICU/PICU setting. This course will focus on understanding the co-morbidities associated with prematurity and medically-complex infants and increase critical reasoning skills for the clinician to create treatment plans to advance motor and feeding skills for the infant. It will build on information provided during the “Baby Beats and Breaths” course but can be a stand-alone course for advanced clinicians working with medically complex infants. This course will provide the clinician with hands-on treatment techniques for positioning/handling infants in the Small Baby unit (infants under 32 weeks gestation), oral motor interventions to support pre-feeding/feeding development, edema management including lymphatic facilitation, post-surgical considerations for infants, trunk facilitation to support cough development, extremity facilitation to support ribcage development, pelvic floor activation for stooling, abdominal facilitation for infants with GI complications, scar assessment/treatment, and feeding techniques for infants with multi-system co-morbidities.

Focus will be on comprehensive assessment and management strategies. Participants will have the opportunity to expand their repertoire of evaluation techniques and intervention strategies when working with infants who demonstrate feeding or swallowing dysfunction. It will span from the hospital/NICU environment through homecare, EI, and outpatients within the first two years of life. This course will be relevant to physical and occupational therapists, speech and language pathologists, lactation consultants, nurses and other health professionals who specialize in or who have an interest in infant feeding disorders. Learning Objectives:
- Evaluate specific infant feeding disorders and generate appropriate treatment strategies for their management.
- Summarize the pathophysiologic principles underlying the suck, swallow, breathe triad and how they apply to breast and bottle feeding.
- Describe increased awareness of diagnostic procedures such as videofluoroscopic swallowing study, infant-focused feeding assessment, evaluation of gastroesophageal reflux.
- Discuss the interplay between specific diagnoses (such as prematurity, gastroesophageal reflux, cleft lip, and palate) and infant feeding dysfunction.
- Select and use treatment strategies to address swallowing dysfunction, aspiration and oral motor dysfunction.
- Synthesize knowledge and utilize best practices in assessment and management of feeding problems in infants.

Medical co-morbidities associated with infants requiring a tracheostomy tube, implications of the tracheostomy tube on oral feeding/motor development, integrating physical rehabilitation treatments and respiratory therapy interventions to maximize infant outcomes with oral feeding, motor development, and daily activities related to play skills will be discussed. Expand your practice by integrating evidence into your clinical decision-making including reading/interpreting vent settings during interventions for objective assessments of infants that are intubated or have tracheostomy tubes. Expanded lecture/lab section devoted to infants with tracheostomy tubes include oral motor, feeding interventions, postural control, manual cough facilitation, positioning/handling, integrating use of a one-way valve into treatment, weaning respiratory support, and durable medical equipment recommendations for infant/family in preparation for home discharge. This course will focus on infants up to 12 months corrected gestational age with information applicable to clinicians working in the NICU, PICU, transitional pediatric care, in-home therapy, early intervention, and outpatient clinics.

Populations range from newborn infants in the Neonatal Intensive Care Unit (NICU) through children in school based & rehabilitation settings. Problem solving/intervention will be addressed through lecture, videos, and case analyses. Functional outcomes are emphasized through targeted intervention strategies based on sensorimotor learning principles and consideration of neural plasticity. Problem areas include: airway (e.g., laryngomalacia, laryngeal cleft, vocal fold paralysis, tracheostomy/ventilator); GI tract (e.g., gastroesophageal reflux, eosinophilic esophagitis); nutrition/hydration; advancing feeding in NICU and beyond to include tongue & lip tie; tube feeding & weaning; barriers to oral feeding & intervention strategies with; picky/finicky eaters. Discussions will include the urgent need for specific measurable outcomes and pros/cons for varied frequency and intensity of direct and indirect interventions/therapies. Learning Objectives:
- Apply knowledge of airway deficits for management decisions with infants and children who demonstrate oropharyngeal dysphagia
- Identify barriers to successful oral feeding (e.g., nutrition, GI tract, and neurologic conditions)

Participants will leave with a comprehensive therapy toolbox including suggestions for reducing mealtime stress and anxiety and ensuring family carryover. Join us for a Train-the-Trainer session on the Early Feeding Skills Assessment (EFS) Tool. Effective feeding care starts with assessment of what the infant needs to be successful. Learn to use the EFS to effectively provide an infant-guided approach to feeding. Simultaneously learn to train others back home to use the EFS to strengthen your unit’s feeding care. Review current research, the role of experience, dynamic systems theory, and feeding outcomes after NICU. Videotapes with enhanced audio of swallowing and breathing to learn key skill areas of the EFS: respiratory regulation, oral motor and swallowing function, physiologic stability, engagement, and change in coordination patterns of s-s-b as infants develop. Gain confidence scoring early feeding skills as not yet evident, emerging or established. Learn components of an infant-guided, co-regulated approach to feeding and contingent adaptations that make this approach so effective, using the EFS to plan individualized interventions. Receive resources to train others to use the EFS. As a group, network and navigate challenging issues and role-model a collaborative feeding practice. Learning Outcomes

VitalStim Therapy and Beyond: DINES (Dynamic Integration of NMES and Exercise for Swallowing) This accredited hands-on competency course will focus on the muscle physiology, kinesiology, and pathology of the normal and abnormal swallowing mechanisms as it changes across infancy, through childhood and into adulthood. The principles of NMES will be taught in tandem with active dysphagia exercise therapy. NMES for Dysphagia protocols, using FDA cleared devices, will be covered against the backdrop of neurophysiology of the swallowing mechanism and available research. Adjustments to NMES parameters – frequency, phase duration, duty cycle, and more – will be incorporated in specific treatment protocols and extensively practiced in hands-on lab sessions. Cases will be evaluated using the impairment model (pathology; symptoms; signs; dysfunctions; and impairments) and treatment planning will be practiced. Participants will receive one-on-one attention from two experienced instructors, a dysphagia expert and an electrical stimulation expert. Instructors will deliver relevant information on FDA clearance and clinical options relevant to different equipment options available today to better equip clinicians in the ever-changing world of NMES for the treatment of pediatric and adult dysphagia. Participants who successfully complete this course will leave as a skilled VitalStim Provider and DINES Provider with the confidence and building blocks needed to implement a successful NMES for dysphagia therapy program.

Learn how to look beyond aspiration and appreciate the entire swallow pathway and its potential for compensations and maladaptations in relation to co-morbidities. Procedures in preparation for and during the study that are most common across the US. Recommendations and optional treatment modalities specific to pediatrics will be highlighted. A pediatric history and report format provided to enhance learning. Videos of normal swallowing and typical radiographic swallowing problems will be shown. Case scenarios, and real-life examples discussed.
