Managing pediatric dysphagia can be challenging as practices evolve, new protocols are implemented, and multiple developmental as well as anatomical factors can impact an infant or child’s ability to eat or drink safely and efficiently. By identifying, assessing, and treating the multiple factors causing or contributing to pediatric dysphagia, BRIDGE provides a multifaceted assessment and treatment approach that provides therapists the tools to think critically when working through difficult cases. The BRIDGE Approach taught in this course provides attendees with information from embryology through childhood and focuses on how the neurological, anatomical, as well as physiological components of growth and development impact pediatric feeding and swallowing. Evaluation to treatment, is presented through the collaborative lens of a speech-language pathologist and an occupational therapist. Different perspectives will be discussed from the pediatric swallowing mechanism from an anatomical and physiological perspective including both instrumental and clinical assessment methods by the SLP to the postural, sensory, and behavioral components to feeding and swallowing including considerations for adaptive interventions when applicable. Attendees will be given strategies to implement immediately when working with children with dysphagia as well as discuss questions, cases, and concerns with the presenters. By BRIDGEing the gap between the multifaceted components of pediatric dysphagia, attendees will gain effective tools to implement when managing pediatric feeding and swallowing disorders.

Evolution of swallowing physiology from infancy forward. Critical information for conducting and interpreting pediatric swallow studies and problem-solving at each juncture. Learn how to look beyond aspiration and appreciate the entire swallow pathway and its potential for compensations and maladaptations in relation to co-morbidities. Procedures in preparation for and during the study that are most common across the US. Recommendations and optional treatment modalities specific to pediatrics will be highlighted. It will focus on advancing the critical reasoning skills of the neonatal therapist to support the infant’s ability to obtain foundational motor and sensory skills. These skills include the ability of the infant to cough, produce audible phonation, oral motor skills, swallow skills, trunk facilitation, gestational age-appropriate positioning/handling to support the emergence of gross motor skills for postural stability. Advance your assessment, treatment, and outcomes for infants requiring a tracheostomy tube. Medical co-morbidities associated with infants requiring a tracheostomy tube, implications of the tracheostomy tube on oral feeding/motor development, integrating physical rehabilitation treatments and respiratory therapy interventions to maximize infant outcomes with oral feeding, motor development, and daily activities related to play skills will be discussed. Expand your practice by integrating evidence into your clinical decision-making including reading/interpreting vent settings during interventions for objective assessments of infants that are intubated or have tracheostomy tubes. Expanded lecture/lab section devoted to infants with tracheostomy tubes include oral motor, feeding interventions, postural control, manual cough facilitation, positioning/handling, integrating use of a one-way valve into treatment, weaning respiratory support, and durable medical equipment recommendations for infant/family in preparation for home discharge. This course will focus on infants up to 12 months corrected gestational age with information applicable to clinicians working in the NICU, PICU, transitional pediatric care, in-home therapy, early intervention, and outpatient clinics. Learning Objectives: 1. Differentiate medical pathology from normal development for infants requiring a tracheostomy tube. 2. Determine how the placement of a tracheostomy tube influences the development of oral feeding and postural control skills. 3. Identify three risk factors for feeding dysfunction associated with oral feeding an infant with a tracheostomy. 4. Apply at least two feeding strategies for infants requiring a tracheostomy tube to improve sensory and motor performance with oral feeding. 5. Apply at least two postural control strategies for infants with a tracheostomy tube to improve their ability to utilize vocal cord movement to enhance their cough, phonation, and gross motor movement. 6. Choose an individualized care plan for an infant using the provided case study with tracheostomy tubes for improved outcomes with oral feeding, cough facilitation, motor control, and phonation.
